We, the parents/legal guardians and students of the incoming class of SY 2020-2021, recognize our duties and responsibilities

with regard to enrollment in De La Salle-College of Saint Benilde (DLS-CSB). In view of this, we jointly understand and agree

to the following:

1. To pay the non-refundable conrmation fee of PhP5,000.00 (cash) which, upon enrollment, shall be credited to the tuition

assessment. In case of withdrawal of conrmation or failure to comply with the submission of complete admission

requirements, the paid conrmation fee is non-refundable and will be charged against administrative and processing costs.

2. To pay the prescribed tuition and other fees (including any increase or fees that may be approved and/or mandated

by the government) according to the DLS-CSB’s payment schedules for the duration of the student’s stay in the College.

3. To abide by the rules and regulations governing the student’s enrollment at DLS-CSB, as stated in the Student Handbook

and/or other College circulars/memoranda/rules, including compliance with Mandatory Drug Testing and the non-fraternity

contract of the College.

4. To actively participate in any parent-college relations programs and activities geared towards the concept that the total

development and education of the student depends on a home and school partnership.

5. To provide accurate student information for ocial school records. Submitted application-related documents found

to be spurious shall be a ground for nullication of the student’s admission to the College.

6. For student with pre-existing Special Education Needs (SEN) who did not disclose and/or comply with the process during

application but was accepted into DLS-CSB, the College is not obliged to provide student with any support or reasonable

accommodation with regards to his/her condition and will be treated as a regular student.

7. For International Students

7.1 Once the student who is a 9(f) visa holder accumulates a total number of seven (7) Rs in his/her academic record,

he/she will no longer be endorsed for student visa extension and will be advised to downgrade his/her visa and transfer

to another school;

7.2 If the student intends to take up a Professional Regulation Commission (PRC) course, he/she is the one responsible

to check with PRC and comply with the “proof of reciprocity” between the Philippines and the applicant’s country of origin

should they want to apply for the board examination;

7.3 The student is only allowed to shift his/her degree program once for the entire duration of his/her stay in DLS-CSB;

7.4 A bond amounting to PhP110,000 is collected on their rst enrollment as proof of nancial capability and as a security

deposit in case of default in payment of tuition and fees. The bond is only a security deposit and will be returned

to the student upon his/her separation from the College. Refund will be in full or partial, depending on his/her account

status at the time of separation from the College.

NOTE: Submission of this signed Statement of Responsibilities to the Admissions Center is one of the requirements for conrmation

of enrollment at De La Salle-College of Saint Benilde.
